While clinical guidelines are still evolving, recent trial data and manufacturer guidance have given us a much clearer picture of how to navigate this transition safely. Here is what you need to know about the dosing protocol when making the switch.

### The Big Question: Do You Have to Start from the Bottom?
When a person first begins a GLP-1 medication, they must undergo a “titration” period. This means starting at the absolute lowest dose and slowly ramping up over several months to allow the body to adapt and minimize gastrointestinal side effects.
Naturally, patients currently on a high maintenance dose of an injectable medication worry that switching to a pill means they will have to reset back to zero and risk stalling their progress.
Fortunately, the latest clinical insights show that if you are already on a high dose of an injectable, you may not need to start at the lowest oral dose.

### Protocol 1: Transitioning to Oral Orforglipron (Foundayo)
Let’s first look at Foundayo (orforglipron). Please note that while Foundayo is made by the same manufacturer as Zepbound, they are entirely different chemical compounds.
Data from the Attain/Maintain trial provides clues regarding safe transitions. In a separate study (the Surmount-5 trial), patients who reached the maximum doses of subcutaneous semaglutide or tirzepatide were switched directly to oral Foundayo to evaluate weight maintenance.
The results were encouraging:
* Patients switched to an oral Foundayo dose experienced very few side effects.
* Based on this, it is reasonable for patients taking higher doses of Zepbound or Wegovy to transition to a higher oral Foundayo dose.
* From that initial dose, providers can then titrate patients up as needed.
Note: If you are currently on a lower dose of Wegovy or Zepbound, your provider will likely choose a lower starting oral dose to protect your stomach.*

### Protocol 2: Transitioning to Oral Wegovy (Semaglutide)
If you are moving from the injectable form of Wegovy to the oral version of Wegovy, the drug manufacturer provides definitive guidance.
* Patients taking the maximum Wegovy injection can switch to a corresponding higher oral dose. You do not need to build up through the lower pill strengths.
* What if you are switching from Zepbound to the oral Wegovy pill? While there is no official direct guidance for cross-drug transitions yet, clinical consensus suggests that if you are on a high dose of Zepbound, transitioning to the higher oral Wegovy pill is a reasonable approach. However, this is entirely dependent on your individual prescriber’s discretion.

### Timeline: When to Take Your First Pill
The timing of your transition is critical to maintaining a steady level of the medication in your system. For both medications, you should start your daily oral dose exactly one week after your last injection. In other words, take your first pill on the exact day your next shot would have been due.
